MatrixCare Integration Instructions
The following article provides an overview of the steps required to integrate MatrixCare with VitalCare Connect.
🚨 IMPORTANT: Only authorized parties from your facility may request an integration between MatrixCare and VitalCare Connect.
Customer Authorization
- Request a Customer Authorization form via your point of contact at VitalCare Connect.
- The form will require the following information from your facility:
- Customer Name and Email
- Customer #
- Vendor Product Offering Name
- Facility Name(s), Address(es), and Care Setting(s)
- MC Billing ID & Applicable Care Setting(s)
- Authorized Signature, printed name, title, email, phone, and date
- Additional notes may be added
- Return the form via email to your VitalCare Connect point of contact.
- Note: processing time for MatrixCare may take up to 30 days, at which point you will be contacted to schedule a go-live date with VitalCare Connect.
