MatrixCare Integration Instructions

The following article provides an overview of the steps required to integrate MatrixCare with VitalCare Connect.

🚨 IMPORTANT: Only authorized parties from your facility may request an integration between MatrixCare and VitalCare Connect.

Customer Authorization

  1. Request a Customer Authorization form via your point of contact at VitalCare Connect.
  2. The form will require the following information from your facility:
    1. Customer Name and Email
    2. Customer #
    3. Vendor Product Offering Name
    4. Facility Name(s), Address(es), and Care Setting(s)
      1. MC Billing ID & Applicable Care Setting(s)
    5. Authorized Signature, printed name, title, email, phone, and date
    6. Additional notes may be added
  3. Return the form via email to your VitalCare Connect point of contact.
  4. Note: processing time for MatrixCare may take up to 30 days, at which point you will be contacted to schedule a go-live date with VitalCare Connect.
